Position: Admissions/Certification RN
Location: Plymouth, MA & Portland, ME
Job Status: Full Time
Responsible for evaluation and assessment of a patient referral to the completion of the initial plan of care for patient referrals from intake to admissions to hospice.

Admissions RN's are responsible for evaluation and assessment of a patient referral to the completion of the initial plan of care for patient referrals from intake to admissions to hospice. Additional responsibilities may include conducting personal visits to patients for re-certification process.  Also, may include activities representing the hospice in  community events.

 

Education:  Graduate of an accredited nursing program, currently licensed as a Registered Nurse (RN) or Nurse Practitioner (NP) in the state and in good standing with the Board.
Bachelor of Science in Nursing and/or certification in hospice preferred.
Experience:  Two (2) years experience in providing medical, surgical, oncology, or hospice nursing
