Position: RN Case Manager
Location: All Locations
Job Status: Full Time
The RN Case Manager provides nursing care and hospice case management to hospice patients and families. Implements nursing interventions and coordinates the Patient/Family Interdisciplinary Plan of Care. Chinese, Russian or Spanish speaking needed.

The RN Case Manager provides nursing care and hospice case management to hospice patients and families.  Implements nursing interventions and coordinates the Patient/Family Interdisciplinary Plan of Care.  Requires the ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention, and family intervention. Requires strong assessment skills, solid teaching skills and the ability to work well with patients and families.   Must have ability to work well with interdisciplinary team and healthcare provider.  Requires excellent written and verbal communication skills.

  • Education:  Graduate of an accredited nursing program, currently licensed as a Registered Nurse (RN) or Nurse Practitioner (NP) in the state and in good standing with the Board.
    Bachelor of Science in Nursing and/or certification in hospice preferred.
  • Experience:  Two (2) years experience in providing medical, surgical, oncology, or hospice nursing
