Position: Liaison Nurse / NP Palliative Care Visits
Location: All Locations
Job Status: Full Time
The Liaison Nurse is responsible identifying and developing on going relationships with specific acute care hospitals and/or long-term care facilities to maintain liaison for the promotion of public awareness and understanding of the hospice program.

The Liaison Nurse is responsible identifying and developing on going relationships with specific acute care hospitals and/or long-term care facilities to maintain liaison for the promotion of public awareness and understanding of the hospice program.  Responsible for processing patient referrals from intake to admission to hospice. Requires demonstrated knowledge and experience in bedside nursing, symptom management, crisis intervention, and family intervention.  Excellent written and communication skills, including public speaking. Requires strong assessment skills, solid teaching skills and the ability to work well with patients and families.   

  • Education:  Graduate of an accredited nursing program, currently licensed as a Registered Nurse (RN) or Nurse Practitioner (NP) in the state and in good standing with the Board.
    Bachelor of Science in Nursing and/or certification in hospice preferred.
  • Experience:  Minimum two (2) years experience in a healthcare services position.
